Berklee Academic Dismissal Inquiry

All academic dismissal inquiries should be based on extenuating circumstances with written verification included. You will be expected to upload a copy of acceptable documentation, medical or otherwise, to corroborate your circumstances.


Your submission will be considered only if you have met the following requirements:

  • You received a notification from Student Success with a link to this form.
  • Your inquiry is in alignment with the upcoming deadlines published on the Enrollment Appeals Timeline.
You will be asked to outline any extenuating circumstances which have impacted your ability to meet all the terms of your Academic Recovery Contract and/or Satisfactory Academic Progress during your most recent semester of enrollment. It's important to note that details of extenuating circumstances predating your most recent semester of enrollment, which may have been included in past SAP Appeal Form submissions, may not be considered.

Documentation Upload
Documentation is required for all appeals. Failure to provide acceptable documentation will result in an automatic denial. Please save all supporting documentation to one PDF file. By uploading documentation to this form, you authorize full release to the committee members for appeal review and consideration.

If you have completed transfer classes for the purpose of reinstatement to the college, please upload a PDF or image screenshot of your grades here.

Examples of Extenuating Circumstances

Examples of Supporting Documentation

A student's physical, mental, or emotional illness, injury, or disability.

Documentation from a medical provider, or a personal statement if medical documentation is not immediately available.

Loss of a family member or close significant person in a student's life.

A copy of an obituary or death certificate and a personal statement.

Illness, accident, or injury of a family member or significant person in a student's life.

Documentation (for instance, a letter from a medical practitioner, a police statement, hospital documents) related to the individual, and a statement from the student about how this incident impacted their studies.

Natural disaster

A personal statement and written documentation of the event (a news article, insurance paperwork, police statement, etc.).

None of the above

A personal statement detailing why you feel that continued enrollment and aid eligibility should be considered for your situation.


Acknowledgements
  • I acknowledge that I have been academically dismissed as a result of multiple semesters of not meeting Berklee's required SAP Standards, as well as failure to fulfill all the terms of my Academic Recovery Contract (if applicable).
  • I acknowledge that my form submission and all relevant student account information will be reviewed and considered by a committee of college officials, who reserve the right to approve or deny my inquiry.
  • I understand that the decisions and recommendations of the Enrollment Appeals Committee are final and cannot be appealed further.
  • I understand that if my inquiry is successful, I must meet with my student success advisor to discuss my academic contract.
  • I understand that if my inquiry is successful, I am responsible for covering my full bill, resolving any check-in restrictions, and checking in by the published deadline.
  • I understand if no documentation is provided, my appeal will be automatically denied.
