all appeals. Failure to provide acceptable documentation will result in an
automatic denial. Please save all supporting documentation to one PDF file. By
uploading documentation to this form, you authorize full release to the committee
members for appeal review and consideration.
If you completed credits for reinstatement, you must upload a PDF of your official or unofficial transfer transcript, including the name/seal of the institution, the semester(s) the courses were completed, and all courses attempted with their final grades. Failure to provide a complete transcript may result in the denial of your appeal.
