Documentation is required for
all appeals. Failure to provide acceptable documentation will result in an
automatic denial. Please save all supporting documentation to one PDF file. By
uploading documentation to this form, you authorize full release to the committee
members for appeal review and consideration.
If you have been suspended from Berklee for at least one fall or spring semester, you must provide evidence of completed transfer classes for the purpose of reinstatement to the college. Students appealing for reinstatement must complete at least 6 credits of coursework, earning a "C"/2.0 or higher in all attempted credits.
