Documentation

Documentation is required for all appeals. Failure to provide acceptable documentation will result in an automatic denial. Please save all supporting documentation to one PDF file. By uploading documentation to this form, you authorize full release to the committee members for appeal review and consideration.

f you have been suspended from Berklee for at least one fall or spring semester, you must provide evidence of completed transfer classes for the purpose of reinstatement to the college. Students appealing for reinstatement must complete at least 6 credits of coursework, earning a "C"/2.0 or higher in all attempted credits. Examples of Extenuating Circumstances

Examples of Supporting Documentation

A student's physical, mental, or emotional illness, injury, or disability.

Documentation from a medical provider, or a personal statement if medical documentation is not immediately available.

Loss of a family member or close significant person in a student's life.

A copy of an obituary or death certificate and a personal statement.

Illness, accident, or injury of a family member or significant person in a student's life.

Documentation (for instance, a letter from a medical practitioner, a police statement, hospital documents) related to the individual, and a statement from the student about how this incident impacted their studies.

Natural disaster

A personal statement and written documentation of the event (a news article, insurance paperwork, police statement, etc.).

None of the above

A personal statement detailing why you feel that continued enrollment and aid eligibility should be considered for your situation.


Acknowledgements

  • I understand that if this appeal is not successful, I will be required to complete requirements as set forth by the Enrollment Appeals Committee for future reinstatement consideration. These requirements may include completing six (6) or more transfer credits at an accredited institution, all with a grade of "C"/2.0 or higher.
  • I understand that the decisions and recommendations of the Enrollment Appeals Committee are final and cannot be appealed further.
  • I understand that if my appeal is successful, I must meet with my student success advisor to discuss my academic contract.
  • I understand that if my appeal is successful, I am responsible for covering my full bill, resolving any check-in restrictions, and checking in by the published deadline.
  • I understand if no documentation is provided, my appeal will be automatically denied.
