New Student Veteran and Dependent Intake Form

The purpose of this form is for Berklee student veterans, dependents, and spouses to securely submit their VA benefit eligibility documentation. This form should only be submitted for in-person "resident" students enrolled on campus at Berklee College of Music, Boston Conservatory at Berklee, Berklee Valencia, and Berklee NYC. If you are an online student please visit https://online.berklee.edu/military/forms to access all military forms for Berklee Online.

Please complete all questions, upload your documents, and submit the form. After registering for classes each semester, you will also have to fill out the Berklee VA Certification Request Form. Berklee will not certify your enrollment to the VA until we have received your certification request.

Transcript Requirement

Per federal VA regulations, Berklee College of Music requires all students utilizing VA educational benefits to provide their official transcript from the Community College of the Air Force or Air University. Berklee will not certify your enrollment without a copy of your transcript. To send Berklee a copy of your official transcript please visit www.parchment.com.
